The situation

What has happened

Band knife 2 in the Brunswick cutting room has been tagged out because its blade guard does not lower fully to lay height. The hazard is in the WHS Incident and Hazard Register at a high rating and is still open. The contractor cannot attend until next week, which means all band knife work is running through knife 1 and the cutting room is under pressure. The Quality and WHS Coordinator wants the risk rating verified against the matrix in the WHS Management Plan, the register entry updated with interim controls and owners, and a toolbox talk on guarding and the blade change process delivered to the cutting room team and recorded.

Your brief. Verify and manage the band knife guard hazard in the register, define interim controls with owners and due dates, prepare and deliver a toolbox talk on cutting room guarding and blade change to the cutting room team, and record it on the Toolbox Talk Record Template with follow-up actions.

What to look for

Evidence guide

Look for a risk rating justified by likelihood and consequence from the matrix, interim controls that are practical for the cutting room, and evidence of genuine consultation with Production. The toolbox talk record must be complete, with attendee names, questions raised and actions with owners. Any hazard raised in the talk must appear in the register.

The student's evidence summary lists every record they created or changed in the systems named above, their notes and the tasks they ticked. Verify it against the deliverables and your own assessment tool.
